I am an in network provider with Horizon, Aetna, Amerihealth and with American Specialty Health Plans.
If you’d like to see what your plan may cover, please submit your insurance information using the form below. I will contact your insurance company on your behalf and verify your acupuncture benefits within 24–48 business hours.
Once verification is complete, you will receive a phone call outlining your coverage, including what your plan covers, your deductible status, and any estimated out-of-pocket costs.

No worries! Many of our patients choose to pay privately. We offer simple, transparent cash rates:
Initial Acupuncture Visit (60–75 min): $120
Follow-Up Acupuncture Visit (45 min): $90
